Collage Rehabilitation Partners Expands Footprint in Georgia, Alabama and North Carolina
Beacon Care Home Health Care becomes Collage Nursing & Homecare Partners
Collage Rehabilitation Partners welcomes Beacon Care to their continuum. This in-home private nursing program focuses on treatment of catastrophic, complex injuries including brain injuries, stroke and neurologic disorders. This acquisition offers expanded services in the states of Georgia and North Carolina and adds the state of Alabama to the Collage Rehabilitation Partners national continuum of care.
The executive and management teams will be working closely with the Beacon Care team to ensure a smooth transition and a continuation of the high-quality clinical services for individuals served and their families. Beacon Care will become part of Collage Home & Community Partners and rebranded as Collage Nursing & Homecare Partners.
Becoming part of the Collage continuum will allow management and staff to continue to focus on the provision of high-quality services to the individuals served, with additional support from Collage rehab in terms of management infrastructure.

Collage Home & Community Rehabilitation Partners announces the acquisition of OT Plus, located in Denver, CO. OT Plus was established in 1984 in response to the need for comprehensive, collaborative and functional home and community rehabilitation. Serving individuals with mild to catastrophic injuries, OT Plus is known for providing a strong coordinated team approach to home and community rehabilitation. OT PLUS will assume the Collage Home & Community Rehabilitation Partners name immediately. Kristi Staniszewski, RPT, co-owner of OT Plus, will assume the position of Clinical Director of the program.
Becoming part of the Collage Home & Community Rehabilitation Partners continuum will allow this program to continue to focus on the provision of high-quality services in the Denver area that they are well known for, with support from Collage in terms of management and infrastructure. Learning Services, A Collage Rehabilitation Partner’s Denver location has utilized OT PLUS as a valuable resource for many years. We look forward to maintaining long-standing relationships that OT Plus has in the Denver community.

We are excited to announce the promotion of Kelly Ramsey, MS, CCC-SLP to Vice President of Collage Home & Community Rehabilitation Partners! Kelly will oversee clinical operations for our Home & Community programs which are located in Texas and Southern California.
Kelly has dedicated her career to providing meaningful therapy that makes a difference in the lives of her patients. Kelly is a Speech Pathologist who graduated from Baylor University with an MS in Communication Disorders. She has worked in acute inpatient rehabilitation, outpatient, acute care, and home and community settings. Her special interest lies in the home and community setting where therapists use activities that are integral to patients’ success , facilitating lasting outcomes in a familiar environment.
As a leader, Kelly puts the needs of others first and helps people develop and perform at a high level. She previously served in manager and director roles at both at TIRR Memorial Hospital as co-leader of the Brain Injury Program and at Rehab Without Walls as Executive Director. Kelly came to Collage Rehabilitation Partners as Program Director in our Texas programs in 2019.
Paoli, PA. June 28, 2022. Collage Rehabilitation Partners announced that Shannon Swick has been appointed to the position of Chief Executive Officer. Shannon has served as Chief Development Officer of Collage Rehabilitation Partners since July of 2020 when ReMed and Learning Services merged, forming a national continuum of post-acute services, now in 11 states. In addition to overseeing the organization’s admissions, marketing, and sales functions, she developed Collage Home & Community Rehabilitation in Texas and Southern California. Swick has played and integral role in the successful acquisitions of the former Restore program in Georgia and Tree of Life in Virginia, working on all aspects of both acquisitions.
Shannon has over 30 years of experience in the post-acute brain injury industry in the areas of development, marketing, sales and operations. She enjoys working with teams to make things happen and her extensive background helps her view and assess things from all angles.
She is dedicated to the highest level of clinical integrity for the people we serve. She is a known leader in the industry through involvement and leadership with FABR, NeuroNet, speaking at industry conferences and publishing journal articles related to outcomes in brain injury. Shannon has a passion for business development, high level outcomes, operational alignment and financial integrity, making her a natural fit for the role of CEO.
Collage Chairman, David Barrass will work closely with Shannon to ensure a efficient and effective transition into her new role. “I am excited to have Shannon transition to the CEO position,” said David Barrass, Collage Chairman. “Her leadership over the past two years and her enthusiasm for and attention to accomplishing our clinical, development and growth goals have been crucial to our success during a volatile time in the healthcare industry.”

This 28-bed program, nationally known for their clinical expertise, will expand Collage’s geographic footprint into the state of Virginia, offering Active Neurorehabilitation and Supported Living Services. The executive and management teams will be working closely with the Tree of Life team to ensure a smooth transition and a continuation of the high-quality clinical services for individuals served and their families, and provide support to staff as moving forward as a Collage Rehabilitation Partner. The Tree of Life name will remain as a Collage Rehabilitation Partner.
Becoming part of the Collage continuum will allow Tree of Life management and staff to continue to focus on the provision of high-quality services to the individuals we serve, with additional support from Collage in terms of management infrastructure. We are delighted that Nathan Zasler, M.D., DAAPM&R, FAAPM&R, FACRM, BIM-C, CBIST will continue to work with Tree of Life, as the Medical Director, during the transition period and beyond allowing continuity for patients, families and other stakeholders.

Paoli, PA: Learning Services, A Collage Rehabilitation Partner, welcomes the former Restore Health Group, located in the Atlanta, Georgia area, to their continuum. The program will assume the Learning Services brand name immediately. This 38-bed program, long known for their expertise with neurobehavioral patients, will expand current clinical capabilities in the state of Georgia, adding Neurobehavioral Rehabilitation and Supported Living Services. The Collage Executive and Learning Services Management teams will be working with the program’s leadership, to ensure a smooth transition for the clients and their families, and provide support to staff as they move forward under their new brand. Incorporating organizational best practices and policies will be a priority across all areas of the organization.
