In July of 2020, ReMed and Learning Services came together to form Collage Rehabilitation Partners. Collage continued to expand its continuum of services by acquiring Tree of Life in Virginia and the former Restore program in Georgia and rebranding that program as Learning Services. Collage Home & Community Rehabilitation Partners expanded from the original two programs in Texas and California through acquisition in Colorado and organic growth in Georgia and South Carolina.

Collage Home & Community Rehabilitation Partners

Providing service in Colorado, Georgia, Southern California, South Carolina and Texas Collage Home & Community Rehabilitation Partners brings intensive neurorehabilitation to the most functional setting possible – the patient’s home and community. Licensed, experienced clinicians complete a thorough assessment of the home to ensure individuals can safely access and utilize key areas. From there, an intensive therapy plan is designed which builds physical and cognitive skills in the home and community. Our patients are taking control of their lives, while their families/caregivers are learning the skills they need to support them. By discharge, families/caregivers  are equipped with the tools and techniques necessary to continue the program as independently as possible.

Learning Services

For over 35 years, Learning Services’ continuum of services has provided exceptional treatment programs that address the individual needs of our clients, families and financial partners. Our comprehensive approach supports the people we serve by providing resources to overcome the challenge of neurologic injury – and build a future that is meaningful and productive for the individual.

With locations in California, Colorado, Georgia, North Carolina and Utah our residential environments are designed to deliver comprehensive therapy in real-world settings. Comprehensive neurorehabilitation, neurobehavioral treatment and supported living programs take place in intensive therapeutic environments and functional rehabilitation to meet the needs of individuals with neurologic injuries.

Day Treatment programs in Colorado, California and North Carolina, providing rehabilitation and support are available Monday through Friday.

Comprehensive neurorehabilitation, neurobehavioral treatment and supported living programs take place in intensive therapeutic environments and functional rehabilitation to meet the needs of individuals with neurologic injuries.

Outpatient, Community and Day Treatment programs in New Jersey and Pennsylvania, provide rehabilitation and support in our clinic sites and an individual’s home and community.

Tree of Life

For over 25 years, Tree of Life has provided exceptional programming with treatment plans that are focused on delivering therapies that restore maximum function and provide individuals with interventions, structures and support necessary to maximize independence. Our comprehensive approach places emphasis on maximizing independence, quality of life and providing resources for return to the community and a meaningful life.

Our campus-based location in Virginia includes three large 8-bed homes. And for individuals who are ready to practice independent living skills, we have a home next door to the campus which is utilized for community re-entry and independent living. Comprehensive neurorehabilitation and supported living programs take place intensive therapeutic environments and functional rehabilitation to meet the needs of individuals with neurologic injuries.
