Home & Community Neurorehabilitation

Serving the greater Raleigh and Creedmoor area Collage Home & Community Program provides active neurorehabilitation in a patient’s home and community for diagnoses including:

Brain Injury, Spinal Cord Injury, Progressive Neurological Conditions, Amputation, Complex Trauma

 


Nursing & Homecare

Serving the Raleigh and Creedmoor area, Collage Nursing & Homecare navigates complex conditions, ensuring patients are able to live the fullest lives possible. Providing comprehensive skilled and non-skilled nursing services for diagnoses including:

Traumatic & Acquired Brain Injuries, Spinal Cord Injuries, Burn & Wound Care, Amputation Support, Neuromuscular Disorders, IV Administration & Maintenance

Serving Raleigh, Creedmoor and surrounding areas
