How to Refer

We are committed to providing services to individuals who meet the established admission criteria.

Candidates are determined to be appropriate for services based upon information obtained during our referral, intake and pre-admission screening processes.

Admission Criteria

  • Candidates appropriate for admission to Collage Rehabilitation Partners include those individuals with acquired and traumatic brain injuries and spinal cord injuries.
  • Candidates are determined to be appropriate for services based upon information obtained in a referral intake process and pre-admission.
  • Candidates are medically stable.
  • Candidates have financial support.
  • Candidates must meet the minimum age requirements as determined by the state licensure regulations for the program being considered.

Our Process

  • Give us a call and we will gather the information necessary to start the referral process. This information may include:
    • Review of injury, diagnosis, and treatment received, as well as a review of pre-injury history and request of current medical records
    • Identification of insurance/funding options. The below outline our primary funding sources:
      • Workers’ compensation
      • Private pay, including settlements and trusts
      • Commercial/private health insurance (case-by-case basis)
      • Military funding (case-by-case veterans and active duty military members)
      • Public funding
        • In certain states, we are able to work with state designated programs. Medicare guidelines do not include post-acute neurorehabilitation services and Collage Rehabilitation Partners is not a Medicare provider.
      • Liens (case-by-case basis)
  • Our directors of admissions will review funding resources to determine eligibility for our program. They have extensive experience in discussing cases and negotiating with insurance and case management representatives.
  • A member of our clinical team will perform a pre-admission screening assessment, which may include: an interview with the candidate and family/caregivers, a review of medical records, and a review of expectations of outcomes for admissions. All parties involved in making the final eligibility decision are encouraged to tour the recommended program prior to admission.
  • A report will be generated, which outlines the findings of the screening and outlines the following recommendations, including type of program and services, recommendations of services, and projected length of stay. If we find that the candidate is not appropriate for our programs, we will attempt to provide resources.
  • Once clinical and financial approval is confirmed we will coordinate and confirm an admission date and assist in arranging all details.
